NOTE REGARDING MAILING OF YOUR PASSPORT(S)

Passport Services will not mail a U.S. passport to a private address outside the United States or Canada. If you do not live at the address listed in the "mailing address", then you must put the name of the person and mark it as "In Care Of" in item # 8. If your mailing address changes prior to receipt of your new passport, please contact the National Passport Information Center.

If you choose to provide your email address in Item #6 on this application, Passport Services may use that information to contact you in the event there is a problem with your application or if you need to provide information to us.

You may receive your newly issued passport book and/or card and your returned citizenship evidence in two separate mailings. If you are applying for both a U.S. passport book and passport card, you may receive three separate mailings; one with your returned citizenship evidence, one with your newly issued passport book, and one with your newly issued passport card.

FEDERAL TAX LAW

Section 6039E of the Internal Revenue Code (26 U.S.C. 6039E) and 22 U.S.C 2714a(f) require you to provide your Social Security number (SSN), if you have one, when you apply for or renew a U.S. passport. If you have never been issued a SSN, you must enter zeros in box #5 of this form. If you are residing abroad, you must also provide the name of the foreign country in which you are residing. The U.S. Department of State must provide your SSN and foreign residence information to the U.S. Department of the Treasury. If you fail to provide the information, your application may be denied and you are subject to a $500 penalty enforced by the IRS. All questions on this matter should be referred to the nearest IRS office.

NOTICE TO CUSTOMERS APPLYING OUTSIDE A DEPARTMENT OF STATE FACILITY

If you send us a check, it will be converted into an electronic funds transfer (EFT). This means we will copy your check and use the account information on it to electronically debit your account for the amount of the check. The debit from your account will usually occur within 24 hours and will be shown on your regular account statement.

You will not receive your original check back. We will destroy your original check, but we will keep the copy of it. If the EFT cannot be processed for technical reasons, you authorize us to process the copy in place of your original check. If the EFT cannot be completed because of insufficient funds, we may try to make the transfer up to two times, and we will charge you a one-time fee of $25, which we will also collect by EFT.

FEE REMITTANCE

Passport service fees are established by law and regulation (see 22 U.S.C. 214, 22 C.F.R. 22.1, and 22 C.F.R. 51.50-56), and are collected at the time you apply for the passport service. If the Department fails to receive full payment of the applicable fees because, for example, your check is returned for any reason or you dispute a passport fee charge to your credit card, the U.S. Department of State will take action to collect the delinquent fees from you under 22 C.F.R. Part 34, and the Federal Claims Collection Standards (see 31 C.F.R. Parts 900-904). In accordance with the Debt Collection Improvement Act (Pub.L. 104-134), if the fees remain unpaid after 180 days and no repayment arrangements have been made, the Department will refer the debt to the U.S. Department of Treasury for collection. Debt collection procedures used by U.S. Department of Treasury may include referral of the debt to private collection agencies, reporting of the debt to credit bureaus, garnishment of private wages and administrative offset of the debt by reducing, or withholding eligible federal payments (e.g., tax refunds, social security payments, federal retirement, etc.) by the amount of your debt, including any interest penalties or other costs incurred. In addition, non-payment of passport fees may result in the invalidation of your passport. An invalidated passport cannot be used for travel.

USE OF SOCIAL SECURITY NUMBER

Your Social Security number will be provided to U.S. Department of Treasury, used in connection with debt collection and checked against lists of persons ineligible or potentially ineligible to receive a U.S. passport, among other authorized uses.

NOTICE TO APPLICANTS FOR OFFICIAL, DIPLOMATIC, OR NO-FEE PASSPORTS

You may use this application if you meet all of the provisions listed on Instruction Page 2; however, you must CONSULT YOUR SPONSORING AGENCY FOR INSTRUCTIONS ON PROPER ROUTING PROCEDURES BEFORE FORWARDING THIS APPLICATION. Your completed passport will be released to your sponsoring agency for forwarding to you.

PROTECT YOURSELF AGAINST IDENTITY THEFT!

REPORT YOUR LOST OR STOLEN PASSPORT BOOK OR PASSPORT CARD!

For more information regarding reporting a lost or stolen U.S. passport book or passport card (Form DS-64), or to determine your eligibility for a passport renewal (Form DS-82), call NPIC at 1-877-487-2778 or visit travel.state.gov.

NOTICE TO U.S. PASSPORT CARD APPLICANTS

The maximum number of letters provided for your given name (first and middle) on the U.S. passport card is 24 characters. The 24 characters may be shortened due to printing restrictions. If both your given names are more than 24 characters, you must shorten one of your given names you list on item 1 of this form.

U.S. passports, either in book or card format, are only issued to U.S. citizens or non-citizen U.S. nationals. Each person must obtain his or her own U.S. passport book or U.S. passport card. The passport card is a U.S. passport issued in card format. Like the traditional U.S. passport book, it reflects the bearer's origin, identity, and nationality, and is subject to existing passport laws and regulations. Unlike the U.S. passport book, the U.S. passport card is valid only for entry at land border crossings and sea ports of entry when traveling from Canada, Mexico, the Caribbean, and Bermuda. The U.S. passport card is not valid for international air travel.

ELECTRONIC PASSPORT STATEMENT

The U.S. Department of State now issues an "Electronic Passport" book, which contains an embedded electronic chip. The electronic passport book continues to be proof of the bearer's U.S. citizenship/nationality and identity, and looks and functions in the same way as a passport without a chip. The addition of an electronic chip in the back cover enables the passport book to carry a duplicate electronic copy of all information from the data page. The electronic passport book is usable at all ports-of-entry, including those that do not yet have electronic chip readers.

Use of the electronic format provides the traveler the additional security protections inherent in chip technology. Moreover, when used at ports-of-entry equipped with electronic chip readers, the electronic passport book provides for faster clearance through some of the port-of-entry processes.

The electronic passport book does not require special handling or treatment, but like previous versions should be protected from extreme heat, bending, and from immersion in water. The electronic chip must be read using specially formatted readers, which protects the data on the chip from unauthorized reading.

The cover of the electronic passport book is printed with a special symbol representing the embedded chip. The symbol port-of-entry areas where the electronic passport book can be read.

Fact Number Fact Detail
1 The DS-11 form is used for first-time U.S. passport applicants, as well as for those not eligible to renew their passport by mail.
2 This form is required for applicants under the age of 16, and for those 16-17, special requirements apply to demonstrate parental consent.
3 Applicants must provide evidence of U.S. citizenship, which can include a previous U.S. passport, birth certificate, Consular Report of Birth Abroad, or naturalization certificate.
4 A government-issued photo identification must be presented by applicants when submitting the DS-11 form.
5 Passport photos that meet specific requirements must accompany the DS-11 form; these must be in color, taken within the last six months, and have a plain white or off-white background.
6 The form must be submitted in person at a passport acceptance facility or agency, often located in post offices, libraries, or county courts.
7 The DS-11 application process includes paying a fee, which varies depending on the type of passport book or card, desired, and the applicant's age.
8 Expedited service is available for an additional fee if the passport is needed sooner than the standard processing time of 6-8 weeks.
9 Special provisions exist for U.S. citizens born abroad and those applying from outside the United States.
10 The DS-11 form cannot be submitted online; however, applicants can fill it out online and print it or complete it by hand in black ink.

Steps to Filling Out DS-11 U.S. Passport application

Applying for a U.S. passport for the first time is an important process that requires accuracy and attention to detail. The DS-11 form is your first step towards obtaining a passport. Completing this form accurately ensures that your application process is smooth and reduces the chances of delays. Here are the steps one should follow to fill out the DS-11 U.S. Passport application form correctly.

  1. Begin by visiting the official Department of State website to access the DS-11 form.
  2. Ensure you do not sign the form until instructed to do so by an authorized agent at a passport acceptance facility or consulate.
  3. Enter your full name as it appears on your birth certificate or legal identification document.
  4. Provide your date of birth, place of birth, social security number, and any other names you have used.
  5. Fill in your mailing address, permanent address, phone number, and email address to ensure you can be contacted regarding your application.
  6. Answer questions about your parental information, including names, dates of birth, and places of birth for both parents.
  7. Include your employment or school information as required.
  8. Answer personal questions honestly, including those about your travel plans, previous passports, and your marital status.
  9. For emergency contact information, provide the name, phone number, email, and address of someone who can be contacted in case of an emergency.
  10. Read the oath and statements at the end of the form carefully. Remember, you will sign the form in the presence of an authorized agent.
  11. Review all the information provided for accuracy and completeness before submitting the form at a passport acceptance facility or U.S. consulate, along with your supporting documents and photographs.

After submitting the DS-11 form and necessary documentation, the next steps involve paying the applicable fees and waiting for processing. Processing times can vary, so planning ahead is crucial. You might need to schedule an appointment for application submission, depending on the location. Following these steps carefully can lead to a successful U.S. passport application experience.

Discover More on DS-11 U.S. Passport application

Who should use the DS-11 U.S. Passport application form?

The DS-11 U.S. Passport application form is for first-time applicants, minors under the age of 16, and applicants who cannot renew their passport by mail. This includes if your previous passport was issued when you were under the age of 16 or it was lost, stolen, damaged, or issued more than 15 years ago.

Can I complete the DS-11 form online?

Yes, you can fill out the DS-11 form online using the U.S. Department of State's official website. Once completed, you must print the form and submit it in person at an authorized Passport Acceptance Facility or Passport Agency. Remember, even though you complete the form online, you cannot submit it electronically.

What documents are required when submitting the DS-11 form?

When you submit your DS-11 form, you need to provide proof of U.S. citizenship, a copy of identification, a passport photo, and the applicable fees. For proof of citizenship, a certified birth certificate, naturalization certificate, or citizenship certificate is acceptable. You'll also need to present a valid government-issued ID. Passport photos must meet strict requirements, so it's important to review these before submission.

Are there any fees associated with the DS-11 U.S. Passport application?

Yes, there are fees for applying for a U.S. passport using the DS-11 form. The fees vary depending on the type of passport service you are requesting (e.g., standard, expedited) and your age (adult or child rates apply). Additionally, if you opt for expedited service or need to submit by mail, extra fees will apply. It's recommended to check the most current fee structure on the U.S. Department of State's website or at the acceptance facility.

How long does it take to get a passport after submitting the DS-11 form?

The processing time for a U.S. passport can vary, typically ranging from 4 to 6 weeks for standard processing. If you need your passport sooner, expedited services are available which can reduce processing time to about 2-3 weeks. Keep in mind that these times can fluctuate, especially during peak travel seasons or unforeseen circumstances, so it's wise to apply well in advance of your planned travel date.

Can I renew my passport using the DS-11 form?

No, the DS-11 form is not for renewals, except in specific cases where the previous passport was issued to a minor or cannot be presented for renewal (lost, stolen, etc.). Adults seeking to renew their passport should use the DS-82 form, provided they meet the eligibility criteria for renewal, such as having a previous passport issued when they were 16 or older, within the last 15 years, and that it is not significantly damaged.

What if I make a mistake on my DS-11 form?

If you make a mistake on your DS-11 form, it's important to correct it before submitting. If you're filling it out online, you can easily edit your information before printing. If you've already printed the form or are using a handwritten form, neatly strike through the mistake and write the correct information clearly. Avoid using whiteout or making excessive marks on the form, as this could cause your application to be delayed or rejected. If the mistake is significant or the form is too messy, it's best to complete a new form.

Common mistakes

Applying for a U.S. passport is a crucial step for citizens planning to travel abroad. A key part of this process is accurately completing the DS-11 U.S. Passport application form. Unfortunately, mistakes can happen, which may delay the issuance of your passport or render your application invalid. Here are six common errors to avoid:

  1. Not using the correct form: Not all passport services require the DS-11 form. The DS-11 is specifically for first-time applicants, those under 16, or applicants who cannot renew their passport by mail. Make sure the DS-11 form is the right one for your situation.

  2. Failing to provide all necessary documentation: Your application must include proof of U.S. citizenship, a valid form of photo identification, a photocopy of that identification, and a passport photo. Leaving out any of these can result in your application being delayed or denied.

  3. Using the wrong type of photo: Passport photos have strict requirements regarding size (2x2 inches), background color (plain white or off-white), and the appearance of the applicant (full-face view, neutral expression). Photos that do not meet these specifications will be rejected.

  4. Providing incorrect information: Double-check all details you enter on the form. Mistakes in your personal information, such as name, date of birth, or Social Security number, can lead to significant delays in processing your application.

  5. Forgetting to sign the form: An unsigned DS-11 form will not be accepted. Remember, if you are a parent or guardian filling out the form for a minor, the minor must be present, but it is the parent or guardian who signs the form if the child is under 16.

  6. Ignoring the fee: There are fees associated with passport applications, and they vary depending on whether you are applying for a passport book, card, or both, and if expedited service is needed. Not including the correct fee can delay the processing of your application.

By paying close attention to these common pitfalls, you can help ensure that your passport application process is smooth and successful, paving the way for your travels outside of the country.

Documents used along the form

When applying for a U.S. Passport using the DS-11 form, several other documents are typically required to complete the process efficiently and accurately. These additional forms and documents serve to verify identity, citizenship, and other pertinent details that are crucial for passport issuance. Below is a list of commonly used documents alongside the DS-11 U.S. Passport Application Form.

  • Proof of U.S. Citizenship: This can include an original or certified copy of a birth certificate, a previous U.S. passport, a consular report of birth abroad, a naturalization certificate, or a certificate of citizenship.
  • Proof of Identity: Typically required to show the applicant's photo, name, and date of birth. Common forms include a valid driver's license, a government or military ID, or a previous U.S. passport.
  • Passport Photo: A 2x2 inch color photo taken within the last 6 months. The photo must meet specific requirements regarding size, background color, and the attire worn in the photo.
  • DS-64 Statement Regarding a Lost or Stolen Passport: If the application is for a replacement of a lost or stolen passport, this form is required to detail the circumstances of the loss or theft.
  • DS-3053 Statement of Consent: In cases where one parent is applying for a passport for a child under 16, this form is used if the other parent is unable to be present, to provide their consent for passport issuance.
  • Proof of Relationship to a Minor: When applying for a passport for a minor, documents such as a birth certificate, adoption decree, or court order establishing custody or guardianship are necessary to prove the relationship between the applicant and the minor.
  • DS-5525 Statement of Exigent/Special Family Circumstances: Required for exceptional cases where one parent/guardian cannot obtain the consent of the other parent/guardian for a child’s passport application due to special circumstances.

Together, these documents support the DS-11 form in providing comprehensive proof of the applicant's eligibility for a U.S. passport. It's essential to consult the latest guidelines from the U.S. Department of State to ensure that all documents meet the current standards and requirements. Proper preparation and submission of these forms and documents can significantly streamline the passport application process.

Similar forms

  • Driver's License Application: Much like the DS-11 U.S. Passport application form, a Driver's License application requires personal information such as full name, date of birth, and address. Both documents necessitate proof of identity and citizenship, for which applicants must present official documents like birth certificates or naturalization papers. Significantly, both forms are integral to obtaining crucial identification documents in the United States, serving as fundamental tools for verifying an individual's identity and legal status.

  • Social Security Card Application (Form SS-5): The Social Security Card application shares similarities with the DS-11 form in its requirement for detailed personal information, including full legal name, date of birth, and citizenship status. Both applications are gateways to key government-issued identification that Americans need for different purposes: the DS-11 for international travel and the SS-5 for employment and accessing government services. Additionally, both processes require evidence of identity through documents like birth certificates or passports.

  • Voter Registration Form: Voter registration forms, while primarily focused on enrolling citizens to vote, bear resemblance to the DS-11 form by requiring personal identification details such as name, address, and date of birth. Both forms are pivotal for civic engagement—one by enabling international travel, and the other by facilitating participation in the democratic process. Although the voter registration form may not require as extensive proof of identity as the DS-11, it nonetheless plays a crucial role in verifying individuals' eligibility and identity within a civic context.

  • Application for Naturalization (Form N-400): The Application for Naturalization (Form N-400) is akin to the DS-11 form in its requirement for comprehensive personal data and proof of identity and citizenship. Applicants for both forms undergo thorough scrutiny of their background, necessitating official documents and evidence to support their claims. While the N-400's primary aim is to grant citizenship, leading to the eligibility for a U.S. passport, the DS-11 is the subsequent step for newly naturalized citizens to obtain that passport—making these forms sequential steps in the journey towards full civic participation and international travel capability.

Dos and Don'ts

Applying for a U.S. Passport requires careful attention to detail, especially when filling out the DS-11 application form. To ensure a smooth process, here are several do's and don'ts to keep in mind:

  • Do use black ink when filling out the application. The clarity and permanence of black ink make it the standard for official documents.
  • Do print clearly and legibly. This can prevent any unnecessary delays caused by unreadable information.
  • Do follow the instructions for each section carefully to ensure that all required information is provided accurately.
  • Do provide truthful and accurate information. False statements can result in severe penalties including fines or imprisonment.
  • Don't sign the application until instructed to do so by an authorized passport acceptance agent. Your signature must be witnessed.
  • Don't use correction fluid or tape. If you make a mistake, it's advisable to fill out a new form rather than trying to correct it.
  • Don't leave any sections blank. If a section does not apply, write "N/A" (not applicable). This indicates that you have seen the question and confirmed it does not apply to your situation.
  • Don't attach photos or documents to the application form until instructed. These are usually collected and attached by the agent during your application appointment.

Following these guidelines can help ensure that the application process goes as smoothly as possible, avoiding common pitfalls that can delay the issuance of your U.S. Passport.

Misconceptions

The DS-11 U.S. Passport application form is essential for those looking to obtain a U.S. passport for the first time or for certain renewals. Despite its importance, there are several misconceptions about this form. Understanding these can help applicants navigate the process more smoothly.

  • Misconception: You can complete and submit the DS-11 form entirely online. While it's true that the form can be filled out online, it must be printed and submitted in person at a passport acceptance facility or agency for first-time applicants, minors, or if the applicant doesn't meet the requirements for renewal via mail. This ensures that all necessary original documents can be reviewed and attached as needed.

  • Misconception: A birth certificate is not necessary when applying with the DS-11 form. Despite what some may believe, a birth certificate or another form of proof of U.S. citizenship is absolutely required when submitting a DS-11 form. This document serves as the primary proof of U.S. citizenship for the majority of applicants.

  • Misconception: You must renew your passport using the DS-11 form if your current passport is expired. This is not entirely true. The DS-11 form is for first-time applicants or those not eligible to use the DS-82 form for renewals. Individuals can often use the DS-82 form to renew their passport by mail as long as their current passport is in good condition, was issued when the holder was age 16 or older, was issued within the last 15 years, and was issued in the current name (or documentation for a legal name change is available).

  • Misconception: Errors on the DS-11 form cannot be corrected after submission. While it’s critical to ensure all information is accurate before submitting the DS-11 form, minor errors can usually be corrected at the passport agency or acceptance facility when noticed. For significant errors after submission, contact the National Passport Information Center for guidance on the correct steps to take.

  • Misconception: The photo requirements for the DS-11 application are flexible. The U.S. Department of State has very strict requirements for passport photos. Photos must meet specific size, background color, pose, and clothing requirements, among others. Failure to comply with these guidelines can result in the rejection of the passport application.
